Cinétique du PSA après prostatectomie totale PSA kinetics after total prostatectomy

چکیده
The prostate specific antigen (PSA) is the best marker of the prostate cancer today although not very specific of this pathology. The dynamic interpretation of this marker always has to prevail over that of overtaking a threshold. With the lack of residual cancer, PSA becomes undetectable by the first month after total prostatectomy: less than 0,1 μg/L. The type of diminution monoor biphasic of the marker depends on the chronology of the takings. Faced with residual cancer, PSA either does not become undetectable or increases after an initial undetectable period. A recurrence is defined by a value of PSA higher than 0,2 μg/L and confirmed on two successive assays. The time of appearance of the recurrence and the PSA doubling time after total prostatectomy have, with the initial clinical stage and the Gleason score, a diagnostic value on the nature of the site of recurrence, local or metastatic.

Implications of PSA kinetics for an adverse pathology after radical prostatectomy.
OBJECTIVE To determine and compare the prognostic value of PSA density, PSA velocity and free/total PSA ratio in predicting adverse pathological findings after radical prostatectomy. MATERIALS We analyzed the medical records of 142 patients who underwent a radical prostatectomy from May 2009 until February of 2011. After exclusion of of them for defined criteria, preoperative PSA and its deri...
متن کاملNucleic acid detection immunoassay for prostate-specific antigen based on immuno-PCR methodology.
BACKGROUND Serum prostate-specific antigen (PSA) concentrations after radical prostatectomy typically become undetectable with the use of current immunometric assay methods. Despite modern surgical techniques, 15%-30% of prostate cancer patients undergoing radical prostatectomy develop a biochemical recurrence during follow-up. Unfortunately, poor analytical sensitivity of standard PSA assays d...
متن کاملUsing prostate-specific antigen doubling time in clinical practice.
Isolated PSA failures after definitive local therapy might precede clinically evident manifestations of recurrent disease by many years [1,2]. These biochemical failures might represent local recurrences, distant recurrences, or both, and accurately determining the site of recurrence early remains a diagnostic challenge. Independent of pretreatment and prostatectomy disease features (i.e. pretr...
متن کاملThe possibility of hormone-mediated PSA derangement in prostate cancer treatment
Background: This study was designed to suggest the possibility of hormone-related derangement in salvage radiotherapy (SRT) after radical prostatectomy in terms of prostate-specific antigen (PSA) control. Materials and Methods: Among 160 consecutive prostate cancer patients who received radical prostatectomy, 34 with SRT between 2004 and 2012 were retrospectively reviewed. The numbers of patien...
متن کاملInfluence of trigger PSA and PSA kinetics on (11)C-choline PET/CT detection rate in patients with biochemical relapse after radical prostatectomy.
UNLABELLED The purpose of this study was to investigate the effect of total prostate-specific antigen (PSA) at the time of (11)C-choline PET/CT (trigger PSA), PSA velocity (PSAvel), and PSA doubling time (PSAdt) on (11)C-choline PET/CT detection rate in patients treated with radical prostatectomy for prostate cancer, who showed biochemical failure during follow-up. METHODS A total of 190 pati...
